For Circles of Care (CoC) Program
This Privacy Policy describes Hood river Valley Adult Center (HRVAC) Privacy Policy for the Circles of Care (CoC) program. This is the policy, procedures, for the collection, use, and disclosure of personal information when using the CoC service. It establishes the rights and responsibilities of the Community Liaison, the volunteers, and the clients of the program. The CoC program is a courtesy program of the HRVAC. Services are upon request, as can be filled by volunteers, and are free of charge.
CoC uses personal information to provide services to older adults who request services, including transportation to medical appointments, transportation to grocery shopping, picking up prescriptions, personal check-ins, and other periodic activities to assist older adults.
CoC is committed to safeguarding the privacy of both our volunteers and clients. We want to assure our patrons that we do not share your personal information with third parties unless permitted. This privacy policy outlines how we collect, use, and protect the information you provide us with.
Information Collection.
CoC collects only the information necessary to provide and improve our services. This includes your name, address, phone number, date of birth, your mobility capacity, any information needed to keep you safe, and emergency contact numbers. We do not sell, rent, or share this information with any third party outside of the CoC program.
How We Use Your Information.
The information collected is used solely for communicating with you as the intended party. CoC does not share or sell your personal information with external parties for marketing or any other purpose. Additionally, no mobile or messaging consent information will be shared with third parties / affiliates for marketing / promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
CoC will use personal data for the following purposes:
To provide and maintain services to support older adults, including arranging of CoC service. Contacts will be limited to gathering information to assist in providing support services, or to respond to questions, cancelations, etc.
To manage your registration as a client of the service. The Personal Data or consents you provide can give you access to different functionalities of the Service that are available to you as a Client.
To contact You CoC uses regular mail, email, telephone calls, or texting (with your permission). CoC does not use push notifications.
HRVAC / CoC does use web based services and does not maintain its own servers. Therefore messages sent to HRVAC / CoC and applications filled out for services are maintained on servers outside of HRVAC.
Texting Disclosure.
By texting the CoC phone number, you agree to receive recurring messages from HRVAC / CoC. There will be no charge from HRVAC / CoC for this method of communication. Messaging frequency depends on your interactions and preferences. You can reply STOP to opt-out of future messages or HELP for more information.
HRVAC / CoC has no third parties / affiliates for marketing / promotional purposes, therefore there will no mobile or messaging consent information will be shared with third parties / affiliates for this purpose.
Disclosure of Your Information.
HRVAC / CoC does not share any client information with third parties for marketing or promotional purposes, or any other purposes. Your personal information is kept confidential and is not disclosed to third parties, except as required by law or with your explicit consent. We may disclose your personal information under the following limited circumstances.
We obtain your consent.
We need to do so to provide the services requested, i.e. contact information is provided to the volunteers providing transportation, etc.
We need to provide information to maintain safety for you, i.e. emergency contact information, mobility, and/or capacity limitations, to volunteers.

When we believe it is necessary to disclose Personal Information to investigate, prevent, or act regarding illegal activities, suspected fraud, potential threats to anyone’s physical safety, or as otherwise required by law.

We transfer Personal Information about you if HRVAC / CoC or its assets are acquired by or merged with another company.
We may share aggregated, non-identifiable information with others without any notice to you, such as the total number of people who have been aided in a specific time, or the total number of contacts, etc.
International Data Transfers.
As HRVAC uses web-based service providers, your Personal Information may be transferred to and processed in locations outside the state of Oregon, or in another country. The laws protecting personal information may differ from those in your jurisdiction. We take steps to ensure that your data is handled securely and in line with this Policy,
Data Retention.
We retain your Personal Information only as long as necessary to fulfill the purposes outlined in this Policy, unless a longer retention period is required or permitted by law. We will also retain and use your Personal Information as necessary to comply with legal obligations, resolve disputes, and comply with funding reporting agreements.
Cookies and Tracking Technologies.
Cookies are small text files placed on your device to collect information about your online activity. HRVAC / CoC does not usecookies or similar tracking technologies.
